Job DetailsDescriptionThe Home Health Intake Coordinator is responsible for receiving the referral, evaluating medical diagnoses and historyfor appropriateness, and scheduling clients referred to Flagler Health+ Home Care. The Intake Coordinator is the pointof contact for providers making referrals to the agency, and coordinates any / all intake activities.Receive referrals from physicians and other healthcare providers (hospitals, skilled nursing facilities, IRF, etc).Initiates intake process, including creation of client chart.Collaborates with Administrator and clinical leadership to assess preliminary eligibility for service.Performs financial eligibility verification.Receives documentation, scans and uploads into new client EHR.Ensures insurance-specific documentation is received and uploaded into client EHR.Conducts welcome calls to new clients and / or their caregiver.Verifies the physician or non-physician practitioner who will follow the client’s home health plan of care.Maintains open and clear channels of communication with referral partnersEducationHigh School Diploma or Equivalent requiredCompletion of a formal Medical Assistant program from an accredited school or an Associates Degree preferredExperience1-year Home Health, Skilled Nursing Facility, Hospice required or Medical Office setting Home Health admissions/intake experience preferredAdditional InformationAbility to evaluate patient history and diagnosis to determine appropriateness for home health.Ability to communicate with physician offices to obtain orders and documentation to substantiate the need for home health.Ability to use electronic health records.Excellent Customer Service Skills and the ability to be adaptable and flexible is requiredCertificates/Licenses/RegistrationLevel 2 AHCA background check required upon hire.Must have good driving record.Valid driver's license and proof of auto insurance required for Motor Vehicles Record (MVR) background check.
