CMS PRIMARY HOME HEALTHCARE LLC is seeking a dedicated and compassionate Health Coach to join our team. As a Health Coach, you will play a crucial role in helping individuals improve their overall health and well-being. Your primary responsibility will be to provide guidance, support, and encouragement to individuals on their journey towards better health. This includes developing personalized health plans based on individual needs and goals, conducting regular check-ins, and offering resources and education to promote healthy lifestyle choices.
