SOCIAL SYNERGY PUBLIC ORGANIZATION (hereinafter - Social Synergy NGO), EDRPOU code 42571900, announces a tender for the provision of services (works) of a case manager for the implementation of the project "Ensuring and strengthening services for better care of children with disability".

The project: "Provision and strengthening of services for better care of children with disabilities" is implemented by the United Nations Children's Fund (UNICEF) and takes place from April 2025 to December 2026.

Main tasks:

• regular assessment of individual needs, resources and level of independence of young people who are involved in supported living;
• development, coordination of implementation and regular updating of individual plans of supported living in cooperation with an interdisciplinary team;
• coordination of providing social, psychological, medical, educational and other services in accordance with individual needs;
• support of young people in the process of adaptation to living in the community and the development of household, social and life skills;
• interaction with legal representatives, service providers, local self-government bodies and other interested parties;
• ensuring compliance with the rights of young people, the principles of inclusion, non-discrimination and decision-making support;
• organization of interdisciplinary meetings, planning and assessment of young people's progress;
• documentation, preparation of reports and recommendations;
• participation in activities to prepare young people for independent living and gradual reduction of the level of support.

Qualification requirements:

Mandatory:

• higher education in the field of social work, social pedagogy, or related specialties;
• with young people, in particular with young people with disabilities or with institutional care experience;
• practical experience of individual support / case management;
• needs assessment and individual work planning skills;
• ability to work in an interdisciplinary team;
• documentation and report preparation skills.
